Chase Health Advance Credit

Payment is due at the time of service. It is our desire to make payment for dental services as easy as possible.

If you have dental insurance, our courteous, trained personnel will assist you in utilizing your insurance benefits. Payment of your estimated balance (the portion of cost not covered by your insurance) is required at time of treatment.

We must emphasize that as dental care providers, our relationship is with you, not your insurance company. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date services are rendered. Should problems arise affecting your timely payment of the account, we encourage you to contact us promptly for assistance.

A service charge will be applied to your account balance after 60 days. A posted return check fee will apply for any returned transaction.

The following forms of payment are accepted:

• Cash
• Debit Cards (with Visa/MC logo)
• Credit Cards - Visa, MasterCard, Discover, American Express, CareCredit
• Checks - with proper identification

Pre-Payment Discount

A 5% pre-payment discount will be given on all treatment plans paid in full prior to the commencement of treatment.

Financing

Financing is available through a third party lender. A credit application will be taken, and upon approval, treatment will commence. Low monthly installments can be arranged to fit any budget. Several options are available as well as interest free financing. Ask our treatment coordinator for more details.

Chase is offering an interest free financing option through their HealthAdvance credit program. There are online applications for both general dentistry and ortho-treatments (Invisalign).
